Legal Name Change
The Legal Name Change Form (LNC) is only applicable when there is no Change of Bond Ownership.
WHO CAN REQUEST A LEGAL NAME CHANGE
An individual can legally change their name by reason of marriage, adoption, divorce, annulment, separation in marriage or legal name change.
If you also wish to Change your Address or Direct Deposit information, please ensure that you submit the Change your Address or Direct Deposit Forms (COADD and/or COADDRP) along with your Legal Name Change form.
HOW TO SUBMIT YOUR REQUEST
- Complete the Legal Name Change Form (LNC)
- Once fully completed, mail the form, along with the applicable documents, to Customer Service’s Transfer & Exchange Unit.
